1. Fistulotomy

Treatments we offer

An Anal Fistulotomy is a surgical procedure that opens and drains an anal fistula, allowing it to recover. An anal fistula is a tiny tunnel (tract) connecting the anal canal to a hollow cavity near the anus. Doctors administer medications to patients to induce sleep or relaxation so that they do not experience pain.

2. Seton Technique

In case there is significant involvement of the anal sphincter muscle (complicated fistula), the doctor inserts a surgical thread (Seton) that is left inside the fistula for some weeks to heal it. This thread helps in draining the fistula and can remain in place for around 6 weeks or longer and further heals the fistula while avoiding the cutting of the sphincter muscles. To cure a fistula with setons, the doctor inserts a tight seton that slowly cuts through the fistula over weeks. Tight seton approach is feasible, but it may require several procedures and frequent visits to the doctor, or the doctor may advise other treatment methods/procedures.

3. Advancement Flap Procedure

This method is considered when there is an involvement of the anal sphincter muscles (fistula passes through the anal muscles) or if the doctor assesses that you may be at a higher risk of incontinence after a fistulotomy procedure.

In this procedure, the fistula is sealed with the help of a skin flap or a piece of tissue that is taken from the rectum (last portion of the digestive tract).

This procedure avoids cutting of the anal sphincter muscles but it has a low success rate.

4. LIFT Procedure

The lift procedure method is used by the doctors when the fistula passes through the anal sphincter muscles and doing a fistulotomy is considered risky.

In this two-step approach, a cut is made above the fistula, and the sphincter muscles are spaced apart. A seton is inserted into the fistula tract that forces the widening of the tract. After a few weeks, the doctor removes the infected tissue in the tract and closes the fistula opening.

5. Bioprosthetic Plug

The Bioprosthetic plug is a cone-shaped plug that is made from animal tissues. It is inserted into the fistula to block the opening of the fistula to close it. The goal of Anal Fistula treatment with the bioprosthetic plug is to eliminate the fistula without affecting incontinence.

6. Laser Therapy

The laser therapy method uses focused light in order to destroy and seal the fistula. The laser technique offers a fixed area of concentration and therefore provides a greater control over the surgery. There are a few advantages of laser therapy over routine procedures:

Firstly, it is less time consuming. Secondly, there is a reduced risk of bleeding and inflammation followed by little or no damage to the surrounding area. Moreover, the laser therapy procedure is painless and patients recover quickly.

7. Non-Surgical Method

Currently, the only non-surgical treatment for anal fistula is fibrin glue. Fibrin is a protein that causes clotting of blood and together with the platelets forms a clot over the wound. The fibrin glue is injected into the tract of the fistula which plugs the wound and eventually causes the surrounding tissue to grow into the tract, closing it. Many people prefer this method because it is painless and safe. However, when compared to surgical treatment methods, it is less effective.

What is Anal Fistula?

A fistula is an abnormal connection between two organs or blood vessels or cavities that usually do not connect. Fistulas can develop anywhere in the body. However, the most common location for a fistula is around the anus.

An anal fistula is a tunnel that forms from the anal canal to the skin near the anus. The anus is the opening in the last part of the digestive system through which the stools exit the body. Inside the anus, there are a number of small glands that produce mucus. Sometimes these glands get clogged and become infected, leading to an abscess (a painful collection of pus that results from a bacterial infection). In most cases, these abscesses develop into a fistula. Multiple fistulas can develop around the anus, causing pain and discomfort. Anal fistula(s) do not resolve by themselves and can be treated medically or surgically.

Anal fistulas surgeries are performed by general surgeons.

Fistula surgery is a safe procedure in which complications are very rare. However, few of the expected complications include an adverse reaction to the anaesthetic drug and severe pain in the anal area which is not relieved by medications. There might be bleeding at the site of the surgery coupled with infection and development of pus in the fistula. In some cases, the wound may take a longer time to heal, causing discomfort. The condition may recur or return after the surgery and may require additional intervention.

Following are a few simple measures that can help in preventing an anal fistula:

  • Eat fibre-rich food 
  • Drink lots of water
  • Keep the anal region clean and dry
  • Avoid straining while passing stools
  • Exercise regularly

Your surgeon will schedule a follow-up appointment around 7 to 10 days after the procedure to check for proper recovery. Most fistulas respond well to surgery and recovery is possible without any complications.
