Medications such as pain killers, corticosteroids and laxatives, etc., are useful in treating low-grade piles. These include over-the-counter (OTC) creams, ointments, and suppositories (medication inserted into the rectum).
Painkillers help in reducing pain and discomfort. Corticosteroids help in reducing redness and swelling. Laxatives help to reduce pressure on the lower rectum and relieve constipation.
Treatments we offer
If piles are not relieved by medications or if your piles are of a higher grade, your doctor may suggest surgical options that may include:
A. Minimal Invasive Procedures : In case of painful piles (haemorrhoids) and bleeding, invasive procedures such as rubber band ligation, sclerotherapy, and coagulation can be performed.
- Rubber Band Ligation (Banding): During this procedure, rubber bands are placed around the base of the piles (haemorrhoids) that help in cutting off the blood supply to the haemorrhoids. It withers and falls off within a week. This method is useful in treating piles that are less than grade 4.
- Sclerotherapy: Sclerotherapy involves injecting a chemical solution in the haemorrhoids. This helps the piles to shrivel up. It is recommended for treating Grade 1 and Grade 2 piles. This treatment serves as an alternative for banding.
- Infrared Coagulation: This procedure uses bursts of infrared light to the haemorrhoid that causes it to bleed, harden and shrivel up eventually. This method is generally used to treat grade 2 and grade 3 haemorrhoids.
B. Surgical Procedures : Surgical procedures are recommended in case of large haemorrhoids that cause pain, discomfort or bleeding.
- Haemorrhoidectomy: In this surgery, the excess haemorrhoidal tissue that is causing the bleeding is removed. The surgery is done under local or regional anaesthesia. It is the most effective way of treating severe or recurring hemorrhoids.
- Haemorrhoid Stapling: This method is mostly used only for the treatment of internal haemorrhoids. Here the haemorrhoids are stapled in place blocking the blood flow causing them to shrink. This method is less painful when compared to a haemorrhoidectomy.
What Are Piles (Haemorrhoids)?
Piles, also known as haemorrhoids, are enlarged blood vessels that can be seen in and around the anus (the opening of your bottom). Blood vessels are naturally present in your anus and they play an important role in normal bowel movements for the excretion of faeces. Piles develop when these blood vessels become enlarged. Piles are more common in adults between 45-65 years of age.
Piles occur as a result of increased pressure in your lower rectum. Hence, the veins around your anus stretch under pressure and tend to bulge or swell. This can happen in chronic constipation or chronic diarrhoea, straining during passing stools, being overweight, lifting heavy objects, etc.
Piles are of two types, internal piles (internal haemorrhoids), and external piles (external haemorrhoids).
- Internal piles: They are present inside your anal canal and cannot be seen by the naked eye but might be visible when they protrude out of your anus.
- External piles: They are visible to the naked eye and are present under the skin around your anus.
There are 4 grades of piles:
- Grade 1: Small inflammations (swelled up area) inside the lining of the anus and it does not come out of the anus.
- Grade 2: Are larger and remain inside the anus. They get pushed outside while passing stools and can return to their position.
- Grade 3: Appears outside the anus and can be pushed back easily.
- Grade 4: Large and remain outside the anus. These cannot be pushed back and they usually require surgical removal.
Internal Piles: Generally do not cause any discomfort. But straining while passing stools can cause bleeding.
External Piles: They can cause several symptoms such as intense pain, itching, bleeding and swelling around the anus.
Your doctor can diagnose external piles through physical examination. Diagnosis of internal piles includes a digital rectal examination (DRE) of the anus and the rectum. This is done by inserting a lubricated, gloved finger in the rectum to feel for any lumps. Your doctor may also use an instrument (proctoscope) to look inside your anal canal.
Postoperatively, you will be advised to have a high fibre diet, drink plenty of fluids, avoid straining too hard while passing stools, stay fit and avoid sitting in one place for long.
